Entry Form

Six nominations will be selected (3 female and 3 male).  The 6 nominees will be photographed, given a ticket to the event and encouraged to attend.  At the event, the 6 nominees photographs will be displayed and people will vote for the Grand Prize Winner. All nominations will get a copy of the photograph displayed, with the final winner receiving the grand prize package worth $500.00!
